The purpose of this accredited qualification is to prepare individuals to provide compassionate care and support to patients and families within home and community settings. It enables learners to assess patient needs, implement care plans, and offer physical, emotional, and palliative support under supervision.

Notional Hours split into:
Self-paced online e-learning, Virtual Classrooms, In-real-life practical days, Implementing Knowledge in the Workplace, Time Spent on Portfolio of Evidence (PoE), Assessment and Feedback Sessions
Home-Based Care Worker Qualification Outcomes
The learners completing the qualification will be able to:

Assess and identify the holistic care needs (physical, emotional, spiritual, and social) of patients and household members, and contribute to developing standardised care plans.

Implement delegated components of care plans by providing direct physical and emotional support to patients in home and community settings.

Educate and empower patients and families on managing health conditions, treatment adherence, hygiene, and healthy living practices.

Refer patients to appropriate health and social services to ensure continuity of care.

Record, report, and maintain accurate information related to care and support activities, in collaboration with the healthcare team.

Support palliative and rehabilitative care by improving quality of life, especially for the elderly, chronically ill, or terminal patients.

Participate in multidisciplinary team meetings and communicate effectively with patients, families, and healthcare workers.

Promote patient dignity, cultural sensitivity, and confidentiality in all aspects of care.
If learners already have a Community Health Worker, Care Assistant, or similar healthcare qualification, chances are that they can be RPL’d for some of the Knowledge Modules in this Qualification.
Home-Based Care Worker - % Time Spent Breakdown
The time spent learning is split into Online Self-Paced Learning (by themselves), virtual classrooms and In-real life sessions (With Us, Diverse Conversations). The time spent on workplace tasks is the time spent with the approved workplace company (With You), which will give the learners the necessary experience to successfully complete this qualification.
As we have not implemented the Home Based Care Worker Qualification just yet, we cannot give an accurate estimation of the % Time Spend Table at this time

Curriculum Content
The Qualification has been structured around the Curriculum Document as a standard from CHIETA and QCTO. For you, as a business entity, to claim your B-BBEE skills development points, learners are required to complete Knowledge Modules (KM's), Practical Modules (PM's) and Workplace Modules (WM's).
Knowledge Modules (KM's)
serve as the foundational learning components that provide learners with the theoretical understanding required for effective performance in the role.

Practical Modules (PM's)
are designed to develop the hands-on skills needed to apply theoretical knowledge in real-world scenarios, bridging the gap between classroom learning and workplace performance.

Implemented Month 2 - 4

PM01: Assess and identify the needs of patients and family/household members, and develop/update a standardised care plan in collaboration with a professional supervisor

PM02: Identify and report on possible needs of patients and family/household members on an ongoing basis

PM03: Implement delegated components of the care plan (Household and patient)

PM04: Educate patients and family/household members regarding the specific health/social conditions, implications, and principles of care (to enable patients to make informed decisions)

PM05: Capacitate patients/family/household members in basic practical care skills

PM06: Record and submit required information as per the schedule
Workplace Modules (WM's)
focus on the real-time application of knowledge and skills in an actual work environment, ensuring that learners are work-ready and capable of operating professionally and ethically within the industry.

Whilst implementing this curriculum, learners can provide much-needed personal care and support services, while gaining skills in home-based healthcare, hygiene, nutrition, and patient well-being.
